This statement provides clarity of the policy for management of personal information (PI) for patients at Victoria Muir – Pelvic Health Practice. Such personal information will be used by Victoria solely for the purpose of management of patient’s case and for correspondence with relevant medical practitioners and health professionals concerned with the same patient.

Victoria will not share PI with any party other than those directly involved in the medical care of each patient. Victoria may contact patients by email with information relating to their treatment, such as patient information leaflets, exercise sheets and videos. Patients will receive text reminders of their appointment times unless they choose to opt out. During COVID19 patients will have the opportunity to access Remote Consultations. No recordings of these sessions will be made, patients will book their own slot through Calendly and once confirmed will use a password to access the session. Documentation of the session will be included in each patient’s case records.

Personal information may be stored securely by Victoria for an indefinite period as per primary care practice. Patients may request a copy of patient records in writing. Victoria will provide these at no charge within one month, to be collected in person for reasons of data security.

Victoria will discuss medical information with a family member who accompanies a patient only with verbal consent of the patient. This will be documented within the patient records for that date. For family members not present, written consent is required by the patient before PI can be disclosed. This will be documented within the patient records. Patients will inform Victoria of any changes of details, for example, contact details, change of GP.

All patients are given the opportunity to consent to these terms before treatment commences. Patients who decline these terms will be given the opportunity to discuss which elements they do not wish to consent to prior to treatment beginning.
